TERMS & CONDITIONS

By booking a service, purchasing a plan, or engaging with Chesapeake Revitalize & Rejuvenate, LLC ("the Practice"), you agree to the following terms and conditions in full.

1. CASH-PAY PRACTICE — NO INSURANCE ACCEPTED

 

Chesapeake Revitalize & Rejuvenate is a cash-pay, direct-pay practice. We do not bill insurance for any services. All fees are the sole responsibility of the patient and are due at the time of booking or service.

 

Superbills are available upon request at no charge and may be submitted to your insurance provider for potential reimbursement. The Practice makes no guarantee of reimbursement. Electronic claim submission is available for an additional fee; only the initial submission is included — no appeals or follow-up will be provided.

2. ALL FEES ARE NON-REFUNDABLE

 

All service fees, consultation fees, package fees, membership fees, enrollment fees, and deposits are strictly non-refundable, with no exceptions. This includes but is not limited to:

 

- Consultation fees

- Procedure and treatment fees

- Package and series purchases

- Membership enrollment fees and monthly dues

- Gift cards and prepaid services

 

No refunds will be issued regardless of whether services were used, partially used, or unused.

3. ALL APPOINTMENTS ARE PREPAID

 

All appointments must be paid in full at the time of booking. Your appointment is not confirmed until payment is received. Failure to complete payment will result in the appointment slot being released.

4. NO PRORATION OR CREDITS FOR UNUSED SERVICES

 

No proration, credits, or refunds will be issued for unused services, unused sessions, unused time, or unused discounts. This applies to all membership tiers, packages, series, and prepaid plans.

5. CANCELLATION POLICY

 

All cancellation requests must be submitted in writing via email to the Practice.

 

- Priority Access & Wellness Plus Members: A minimum 6-month commitment is required. Cancellations require 30 days written notice after the minimum commitment period is met.

- Elite Members: Annual prepaid membership. No cancellations or refunds after enrollment.

- All cancellations are effective 30 days from the date written notice is received.

 

Failure to provide proper written notice may result in continued billing.

6. SERVICE EXPIRATION DATES

 

Certain packages and series carry strict expiration dates. Services not used within the specified timeframe will be forfeited without refund or credit. Expiration dates are as follows:

 

- 90-Day Packages: Must be used within 90 days of purchase date.

- 12-Month Series: Must be used within 12 months of purchase date.

- Consultation Credit: The $150 consultation fee is credited toward the O-Shot® or Clitoxin™ procedure only if the procedure is scheduled and purchased within 60 days of your consultation date. This credit applies exclusively to O-Shot® and Clitoxin™ procedures and does not apply to any other services. Credits are non-transferable and cannot be applied after the 60-day window has passed.

 

No extensions will be granted.

7. RESULTS DISCLAIMER

 

Individual results vary. The Practice makes no guarantees regarding specific outcomes, results, or timelines for any treatment, procedure, or service. All treatments are performed based on your individual health history, candidacy, and provider assessment.

8. CONSULTATION REQUIREMENT

 

Certain services require a mandatory consultation prior to booking. Standalone or un-vetted treatment room booking is strictly prohibited for designated services in order to preserve patient safety and candidate eligibility. A list of services requiring consultation is available upon request and on our website.

9. ANTI-FRONT-LOADING POLICY

 

Members who utilize included visit credits and then cancel their membership before fulfilling the minimum commitment period will have those visits repriced at the non-member rate. The difference will be charged prior to membership cancellation being processed.

10. BOOKING & SCHEDULING

 

- The Practice reserves the right to reschedule appointments due to provider availability, emergencies, or unforeseen circumstances.

- Late arrivals may result in a shortened appointment or rescheduling at the Practice's discretion.

- Repeated no-shows or late cancellations may result in prepayment requirements or dismissal from the Practice.

11. SCOPE OF SERVICES

 

Chesapeake Revitalize & Rejuvenate provides wellness, regenerative, and aesthetic services. We are not an emergency medical facility. In the event of a medical emergency, call 911 immediately.

12. HIPAA & PRIVACY

 

Your health information is protected under HIPAA. Our Notice of Privacy Practices is available upon request, on our website, and in our office. Annual acknowledgment is required for active patients.

13. GOVERNING LAW

 

These Terms & Conditions are governed by the laws of the State of Maryland. Any disputes shall be resolved in the jurisdiction of Wicomico County, Maryland.

14. UPDATES TO TERMS

 

The Practice reserves the right to update these Terms & Conditions at any time. Continued use of our services following any updates constitutes acceptance of the revised terms.
